Tri-fold card contains voting ballot for five battles; Batman vs Captain America, Hulk vs Superman, Superboy vs Spider-Man, Wolverine vs Lobo, and Wonder Woman vs Storm, which was to be dropped off at your "Versus Voting Canter" (local comic book specialty store), and lists alternate means of voting by mail or e-mail. All "ballots" were to be recieved by December 31, 1995.
