This issue reprints in its entirety Vault of Horror (EC) #32. The original cover art for the issue, which had been censored to remove the depiction of a meat cleaver stuck in the man's head, has been restored to its original state.
"Whirlpool"
A mental ward patient has delusions of a trio of tormentors, subjecting her to the worst torture. She awakens to the faces of three care-givers, the same three from her nightmares. 8 pages.
"Out of His Head!"
Alex and Stanley discover a mine loaded with gold while camping. Alex buries a hatchet in Stanley's head to avoid sharing the riches. Later, Alex cannot stop having visions of Stanley, coldly staring at him with the hatchet still in place, and it drives him to his end. 7 pages.
"Crash Landing!"
A man perpetrates a murder in mid-air, killing the pilot and bailing out to make is seem like an accident. The plane crashes in the trees, but Fate is in the wind, as his parachute blows him directly over the crash. He is impaled on the broken branches above the crash. 2 pages, text only.
"An Ample Sample"
The neighbor comes over to return tools to his friend and comes to confess that his wife's love of chocolate, and her ensuing obesity had driven him to the breaking point. He invites his friend over to see his work, an enormous chocolate box to serve as a casket for the cup-sized pieces of his wife. 6 pages.
"Funeral Disease!"
The caretaker is trying to save money to make sure that his funeral is a proper one. His employer is in a pinch for cash, and demands that his caretaker loan him his nest egg. When the caretaker refuses, the employer has him robber and murdered. The employer is visited that night by the ghost of his caretaker, who returns the favor. As a fitting ending, the caretaker swaps his corpse for the employers, getting the burial that he deserves. 7 pages.
